-- Legal name or any other names used.

--Correct permanent address

Photograph and any one of the following documents.

1. Passport
2. Pan Card
   
3. Voter’s Identity Card
   
4. Driving Licence.
   
5. Job Card issued by NREGA duly signed by an officer of the State Government
   
6. Letter issued by UIDAI containing details of name, address and aadhar number
   
7. Photo Identity Card (subject to bank’s satisfaction)
   
8. Letter from a recognized public authority or public servant verifying the identity and residence of the customer to the satisfaction of the bank.

7. Any document which provides customer address information to the satisfaction of the bank will suffice.

1. If the address on the document submitted for identity proof by the prospective customer is same as that declared by him/her in the account opening form, the document may be accepted as a valid proof of both identity and address.
2. If the address indicated on the document submitted for identity proof differs from the current address mentioned in the account opening form, a separate proof of address should be obtained.

Accounts of companies.

-- name of the company.
-- principal place of business.
-- Mailing address of the company
-- Telephone /Fax number

1. Certificate of incorporation, Certificate of commencement of business and Memorandum & Articles of Association (up-to-date) certified by the Secretary/Director of the Company.
   
2. Resolution of the Board of Directors to open an account and identification of those who have the authority to operate the account.
   
3. Power of Attorney granted to its managers, officers or employees to transact business on its behalf.
   
4. Copy of PAN allotment letter
   
5. Copy of Telephone Bill.
   
6. List of present Directors signed by the Chairman of the Company.
   
7. List of Authorised signatories with their signatures attested by the Chairman of the Company.
   
8. As a proof of current address at least one of the address proof of each Director and photograph of the Directors.

Accounts of Partnership firms.

--- Legal name
--- Address
--- Names of all partners and their addresses
--- Telephone numbers of the firm and partners.

1. Registration certificate, if registered, with a copy certified by the partners.
   
2. Partnership Deed
   
3. Power of Attorney granted to a partner or an employee of the firm to transact business on its behalf.
   
4. Any officially valid document identifying the partners and the persons holding the Power of Attorney and their addresses.
   
5. Telephone bill in the name of firm/ partners.
   
6. Partnership letter in Bank’s format.

Accounts of Trusts & foundations.

-- Names of trustees, settlors,    beneficiaries and signatories.
-- Names and addresses of the    founder,the managers/directors and    the beneficiaries.
-- Telephone/fax numbers.

1. Certificate of registration, if registered.
   
2. Power of Attorney grated to transact business on its behalf
   
3. Any officially valid document to identify the trustees, settlors, beneficiaries and those holding Power of Attorney, founders/managers/ directors and their addresses.
   
4. Resolution of the Managing body of the foundation/ association for opening and operating the account signed by all the trustees.
   
5. Telephone Bill.
   
6. The trust Deed for verification with copy certified by the Chairman. (The deed should empower the trustees to open Bank Account)
   
7. A list of the names of trustees with their signatures attested by the Chairman.

Accounts of proprietorship concern.

-- Proof of the name, address and     activity of the concern.

1. Proprietorship letter in Bank’s format, besides identity proof & address proof.
   
2. Power of Attorney if granted by the proprietor.
   Any two of the following documents:

4. Certificate/Licence issued by the Municipal Authorities under Shop & Establishment Act.
   
5. Sales & Income Tax Return
   
6. CST/VAT certificate
   
7. Certificate/ registration document issued by Sale Tax/Service Tax/Professional Tax Authority.
   
8. License issued by the Registering authority like Certificate of Practice issued by the Institute of Chartered Accountants of India, Institute of Cost Accountants of India, Institute of Company Secretaries of India, Indian Medical Council, Food & Drug Control Authorities
   
9. Any registration/ licencing document issued by the Central Govt. or State Govt. authority/Department.
   
10. Importer Exporter Code issued to the proprietorship concern by the Office of the Directorate General of Foreign Trade.

Accounts of Salaried employees

Identity card from employer and or employer letter is/are obtained only from corporate and other entities of repute, in addition to at least one of the officially valid documents of identification like, passport, PAN Card, voter identity Card, Driving Licence etc.

Hindu Undivided Family account

1. Declaration from Karta,
   
2. Joint Hindu Family letter signed by the Karta and all major co-parceners.
   
3. PAN allotment letter for verification with a copy.
   
4. Telephone Bill for verification with a copy.

Account of Society, Association, Club, Schools& Colleges.

1. Certificate of Registration for verification with a copy certified by the Chairman/ Secretary, if registered.
   
2. Memorandum of Association (where applicable) certified by the Chairman/Secretary.
   
3. A copy of Resolution of Governing Body, Board of Directors or Managing Committee of the Society authorizing opening and operation of the account, certified by the Chairman of the meeting.
   
4. A list of Authorised signatories with their signatures attested by the Chairman/ Secretary.
   
5. PAN allotment letter for verification with a copy.
   
6. Copy of Bye Laws, Rules, Regulations certified by the Chairman/Secretary.
   
7. Telephone Bill for verification with a copy.

Trust Accounts

1. Certificate of Registration for verification with a copy certified by the Chairman.(for public Charitable trust).
   
2. Trust Deed for verification with a copy certified by the Chairman. (The deed should empower the trustees to open Bank account)
   
3. Resolution for opening and operating the account signed by all the Trustees.
   
4. Any document listing the names of the trustees/settlers/beneficiaries.
   
5. A list of the names of trustees with their signatures attested by the Chairman.
   
6. PAN Allotment letter for verification with a copy.
   
7. Telephone bill for verification with a copy.
