The retired employees willing to be member of the ‘Scheme’ may send their applications in the prescribed format in triplicate through the Branch Manager so as to reach the Chief Manager, D&IR Division, UBI, H.O. on or before 20.04.2012. The prescribed yearly premium of Rs.3,500/- is to be deposited to the credit of A/c. No. 0389050192231 in the name of Bajaj Allianz General Insurance Co. Ltd. maintained with Bank’s Old Court House Street Branch with the compulsory narration in particular column “PRM-GMC-RTD/SPF No.- /Name of the Retired Employee” through the Branch where they have been maintaining accounts and/or drawing pension. Payment of premium in the form of Cheque / Draft / Pay Order will not be accepted. Detailed circular dated 30.03.2012 is available with the branches.
